As a community partner, healthcare team members can support the LTI community by referring interested participants, sharing resources and supporting a client's self managment goals. Healthcare providers can support individuals living with HIV, living with or cured from Hep C, or practicing Harm Reduction achieve their self-management goals. By helping clients get their health care under control, healthcare providers are helping pave the way for clients to pursue other personal or professional goals. From better understanding their lab work to achieving undectable viral loads, health care teams can help LTI graduates with goal setting by using this SMART goals framework and referring interested individuals to the LTI 3-part, 10-day Foundational Training Series.

The LTI Resource Center offers a range of resources for individuals living with HIV, living with or cured from Hep C, or practicing Harm Reduction, their loved ones and community partners. From locating a testing center to obtaining health insurance to understanding the basics of HIV and labwork terminology, healthcare providers can share the LTI Resource Center with clients and refer interested individuals to the LTI Foundational Training Series.

Using evidence based strategies in a peer-to-peer environment, past LTI program participants credit the training programs with helping them better manage their chronic conditions, achieve goals and communicate more effectively with their healthcare providers.
